The Tolstoy Foundation Rehabilitation & Nursing Center is a not-for-profit skilled nursing and rehabilitation center located at 100 Lake Road in Valley Cottage, NY. With roots in the Tolstoy Foundation community, the facility has served the area since around 1970. The 96-bed center supports both short-term rehabilitation patients and long-term residents from Rockland and nearby counties. Set on a hillside campus, it provides care in a setting that reflects its longstanding community mission.
